2010 Registration for the Del Val Dawgs Football and Cheerleading Squads
If you are a NEW Player, and reside in the townships of Alexandria, Frenchtown, Holland, Kingwood, Milford or Lambertville you will be required to complete the forms below, and return them in person on the Registration Dates of March 20 or 22nd. A copy of a Birth Certificate for our files, along with an Original Birth Certificate to view will be required with the registration forms.

All players are required to submit both sides of the completed 2010 POP WARNER Medical Form, dated in 2010. Any forms with a 2009 date are not acceptable. The Medical Form is required before the first day of practice, August 2rd, 2010.
Also, all players are required to submit two (2) copies of their final 2009/2010 report card.
All Paperwork is required by the first day of practice.
No Equipment will be distributed to players with missing and/or incomplete forms.
2010 Registration Fees are as follows:
Work Bond of $100.00 per Family
Flag Football and Cheer$125.00Please submit 2 checks (1 for reg.fee &
Tackle Football $150.001 for the work bond.)
Also, a Mandatory $100.00 Fund Raiser will be req. more to follow.
